Manage your own client portfolio with an established community care provider
Quality Health Care is seeking an experienced Care Coordinator – Home & Community Care to join our Hurstville-based team.
Established in 1999, Quality Health Care is a family-owned and operated provider of disability, home and community care services across Greater Sydney.
This is an office-based role with responsibility for managing a portfolio of clients, coordinating their services and building strong relationships with clients, families, support workers and external stakeholders.
You will play a key role in ensuring each client receives the right support, from the right person, at the right time.
What we offer
- $85,000–$95,000 + superannuation
- Full-time, Monday to Friday + oncall (shared across the team)
- Paid on call allowance

About the role
As a Care Coordinator, you will manage an allocated portfolio of clients and coordinate the services and support workers required to meet their individual needs.
You will be a central point of contact for clients and families and work closely with support workers, clinicians, case managers and other stakeholders.
Your responsibilities will include:
- managing client intake,
onboarding and service commencement
- building strong relationships with clients, families and representatives
- coordinating individual support arrangements and service schedules
- conducting client reviews and home visits
- working with workforce teams to ensure service coverage
- matching support workers with clients based on skills, experience and compatibility
- responding to changes in client circumstances or service requirements
- monitoring service utilisation and identifying emerging issues
- maintaining accurate client information in Virtual Care
- escalating clinical concerns, incidents, complaints and safeguarding matters appropriately
- contributing to quality improvement and service development activities
Our clients have diverse needs and may receive services through NDIS, disability, private care, home and community care, ageing-related or brokered care arrangements.
About you
You may currently be working as a:
- Care Coordinator
- Home Care Coordinator
- Client Services Coordinator
- Community Care Coordinator
- Disability Services Coordinator
- Case Coordinator
You will ideally have previous experience in NDIS, disability, home care, aged care, healthcare or community services.
We are looking for someone who is organised, responsive,
confident managing competing priorities, can build relationships and has good communication skills.
You will bring:
- experience coordinating community-based client services
- strong client and family relationship-management skills
- experience managing a client portfolio or caseload
- confidence coordinating support workers and service delivery
- excellent communication and problem-solving skills
- strong attention to detail
- an understanding of person-centred care
- confidence using client management and rostering systems
- a current Australian driver's licence
Experience using Visual Care or a similar community care management system will be highly regarded.
A qualification in Community Services, Disability, Aged Care, Health or a related discipline is desirable.
