DPV Health is seeking a Senior Lifestyle Program Care Coordinator to deliver care coordination assessments, education and support for clients with frailty in Melbourne's northern growth corridor.

You will coordinate the Senior Lifestyle Victoria Program, identify needs, develop individual care plans, and connect clients with local services while liaising with GPs and other providers to improve health outcomes.
