We are looking for an experienced Support Coordinator with mental health experience to join our team, on a part-time basis with opportunity to move to full time.
This role is best suited to someone who:
Is confident working independently
Can manage their own caseload and priorities
Communicates clearly and follows through on tasks
Is comfortable working with complexity and uncertainty
Key Responsibilities
- Support participants to understand and implement their NDIS plans
- Coordinate services and liaise with multiple providers
- Attend virtual and face to face meetings
- Maintain accurate and compliant case notes and documentation
- Support participants across a range of presentations, including, but not limited to, psychosocial, neurodivergent, and physical disabilities
- Navigate crisis situations and system barriers where required
What We’re Looking For
- Experience in Support Coordination and/or Recovery Coaching (1+ years preferred)
- Strong understanding of the NDIS
- Ability to manage a caseload independently
- Strong communication and documentation skills
- Valid driver’s licence and access to a vehicle
