Stakeholder and Service Coordination

The Community Worker will work collaboratively with the participant's broader support network to assist with coordinated and consistent service delivery.

This may include attending participant care-team meetings, stakeholder meetings, service reviews and other relevant discussions with Support Coordinators, families, allied health professionals, healthcare providers, behaviour support practitioners, community organisations and other disability service providers.

The Community Worker will assist with identifying service gaps, sharing relevant information within appropriate consent and privacy requirements, implementing agreed recommendations and supporting participants to remain connected with appropriate disability, health, community, social, educational and other services relevant to their individual needs and goals.

Staff Training and Support

The Community Worker assists Tayla’d Care Services to maintain consistent and person-centred service delivery by supporting the education and development of support staff.





This may include participant-specific inductions, support-plan training, risk-management training, practical demonstrations, staff handovers and communicating changes to participant support requirements.

Where specialised or clinical training is required, the Community Worker will work within their scope of practice and assist management to coordinate training with appropriately qualified professionals.

Documentation and Reporting

The Community Worker is responsible for maintaining accurate and timely records of supports provided, participant progress, identified risks and relevant observations.

Documentation may contribute to the development and review of participant support plans, individual risk assessments and home safety assessments/checklists and assist Tayla’d Care Services and relevant members of the participant's support team to understand progress, emerging needs, risks and barriers to achieving identified goals.
