What You're Will Be Doing
Provide high-qualitynursing care and home visitswithin the community
Completeclient intake, comprehensive nursing and risk assessments
Develop, implement and regularly reviewindividualised care plans
Administer medications, treatments and other nursing interventions
Coordinate care with clients, families, Support Workers and other healthcare professionals
Provide clinical guidance, education and competency assessments to staff
Review incidents and ensure appropriate clinical follow-up
Maintain accurate and timely clinical documentation
Conductquality checks, clinical audits and compliance reviews
Support compliance acrossNDIS, Support at Home, DVA and other community programs
Proactively identify opportunities to improve clinical quality and service delivery
What We're Looking For
CurrentAHPRA registration as a Registered Nurse (Division 1)
Minimum2 years' experiencein community nursing, disability and/or aged care
Robust experience inclinical assessments and care plan development
Understanding ofclinical compliance, quality control and auditing
Highly autonomous, organised, proactive and self-driven
Ability to manage a community caseload with minimal supervision
Excellent communication and clinical judgement
Ability and willingness to undertakehome visits
Current driver's licence
