Part-Time or Casual position

What You're Will Be Doing
Provide high-quality
nursing care and home visits
within the community
Complete
client intake, comprehensive nursing and risk assessments
Develop, implement and regularly review
individualised care plans
Administer medications, treatments and other nursing interventions
Coordinate care with clients, families,
Support Workers and other healthcare professionals
Provide clinical guidance, education and competency assessments to staff
Review incidents and ensure appropriate clinical follow-up
Maintain accurate and timely clinical documentation
Conduct
quality checks, clinical audits and compliance reviews
Support compliance across
NDIS, Support at Home, DVA and other community programs
Proactively identify opportunities to improve clinical quality and service delivery
What We're Looking For
Current
AHPRA registration as a Registered Nurse (Division 1)
Minimum
2 years' experience
in community nursing, disability and/or aged care
Strong experience in
clinical assessments and care plan development
Understanding of
clinical compliance, quality control and auditing
Highly autonomous, organised, proactive and self-driven
Ability to manage a community caseload with minimal supervision
Excellent communication and clinical judgement
Ability and willingness to undertake
home visits
Current driver's licence
