You'll provide clinical leadership and operational oversight, support complex member cases, coach and develop team members, and contribute to continuous improvement initiatives that enhance service delivery and member outcomes.

What will you do?
Lead, coach and develop a multidisciplinary team to deliver exceptional care coordination and health management services.
Provide clinical leadership and guidance on complex member cases, escalations and service delivery decisions.
Conduct telehealth assessments, health coaching and care coordination activities as required.
Monitor team performance, quality, productivity and compliance with clinical and organisational standards.
Support onboarding, performance management, employee engagement and professional development initiatives.
Manage provider relationships, service quality activities, complaints, audits and continuous improvement projects.
Collaborate with internal and external stakeholders to enhance member and provider experiences.
What are we looking for?
Current unrestricted registration as a Registered Nurse (Division 1) with AHPRA.
At least 5 years' clinical experience within acute, community health,



care coordination or chronic disease management settings.
Previous leadership, team leader or senior clinical experience, including coaching and performance management Strong clinical assessment, decision-making and problem-solving skills.
Experience in clinical governance, risk management and quality improvement Excellent communication and stakeholder management skills.
Solid organisational skills and the ability to balance competing priorities.
Experience in telehealth, health coaching or discharge planning is highly regarded Health insurance, community health or chronic disease management experience is highly regarded Experience leading teams through change and service improvement initiatives is highly regarded Who are we?
