We are looking for an
experienced Support Coordinator with mental health experience
to join our team, on a part-time basis with opportunity to move to full time.
This role is best suited to someone who:
Is confident working independently
Can manage their own caseload and priorities
Communicates clearly and follows through on tasks
Is comfortable working with complexity and uncertainty
Key Responsibilities
Support participants to understand and implement their NDIS plans
Coordinate services and liaise with multiple providers
Attend virtual and face to face meetings
Maintain accurate and compliant case notes and documentation
Support participants across a range of presentations, including, but not limited to, psychosocial, neurodivergent, and physical disabilities
Navigate crisis situations and system barriers where required
What We're Looking For
Experience in Support Coordination and/or Recovery Coaching (1+ years preferred)
Strong understanding of the NDIS
Ability to manage a caseload independently
Strong communication and documentation skills
Valid driver's licence and access to a vehicle
Highly Valued:
Experience working with: Neurodivergence (ASD, ADHD) Complex trauma and mental health
Lived experience (optional and welcomed)
A calm, grounded approach in complex situations
Hybrid role (work from home + community-based support)

Fortnightly pay + superannuation
