Apunipima Cape York Health Council is a community controlled Aboriginal and/or Torres Strait Islander health service. Servicing the communities of Cape York, with the main office is located in Cairns, Apunipima delivers public health care, and social and emotional well-being care, to community members. Being a part of the team at Apunipima means helping us deliver essential services as well as building capacity in communities to manage their own health care needs.
About the Opportunity
Special/equal opportunity measure
This position constitutes a special/equal prospect measure under section 105 of the Anti-Discrimination Act 1991 (Qld). The position is therefore only open to Aboriginal and/or Torres Strait Islander applicants.
The Elder Care Support Connector is resonsible for connecting with local Elders and older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services.
Pre-employment screening for vaccination against Vaccine Preventable Diseases will be conducted for the successful applicant.
Mandatory Requirements
- Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement
- Current Queensland Drivers Licence
- National Police Check within the last 3 months or willing and able to obtain prior to commencement
- Certificate IV in Community Services or Certificate IV in Community Development
About You
To be successful in this role, you will demonstrate:
- A minimum of two (2) years' experience as a social and community worker or in a similar position
- Experience in providing support to Aboriginal and Torres Strait Islander people, preferably in remote communities
- Ability to assist and support clients to access services, reduce service complexity and confusion
- Understanding of My Aged Care and operational guidelines or the ability to quickly acquire
