Aveo is seeking a Care Partner (Clinical) to support residents in their homes across Aveo communities. You will assess care needs, explain options, and create personalised plans while coordinating services to help residents live independently longer.

You'll work with GPs and allied health specialists, maintain records in Alayacare, and deliver compassionate, customer-focused care throughout weekday hours.
