The Role

Join the Tweed Clinical Care team in a rewarding and rapid-paced role as a Care Coordinator and Nurse Mentor. This full time role reports to the General Manager, with support and guidance from the Clinical Care Manager whilst Supervising and supporting our passionate care team.The key purpose of this role is to ensure that all our residents within our beautiful facility in Tweed receive the highest quality of clinical care. The Clinical Care Coordinator is responsible for the management and resourcing of the care needs within the facility, documentation and assessments are up to date and in alignment with best-in-class clinical practices.

Key Responsibilities
Provide strategic and operational leadership to the clinical care team to ensure high‑quality, resident‑centred care.
Oversee the development, implementation,



and regular review of individualised care plans aligned with best practice.
Monitor clinical activities, including mealtimes and key daily events, to maintain consistent service excellence.
Mentor and support RNs, ENs, and PCWs to strengthen clinical capability, teamwork, and skilled growth.
Collaborate with multidisciplinary team including; allied health, GPs, and hospitals to coordinate holistic resident care.
Facilitate effective communication processes such as shift handovers, clinical huddles, and team meetings.
Provide oversight of clinical decision‑making, ensuring compliance with evidence‑based practice and regulatory standards.
Manage complex resident care needs, including specialist referrals, case reviews, and family consultations.
Lead onboarding, training, and ongoing development of care staff, identifying skill gaps and supporting performance management.
Monitor resident health changes, elevate concerns promptly, and implement strategies to minimise risks such as falls and infections.
