The Department of Health Tasmania seeks a Clinical Nurse Consultant for the Rapid Access Service pilot in Sydney CBD, with primary locations in Launceston and Burnie. You will provide senior clinical leadership, assess, triage and support older people with BPSD in residential aged care settings.

Working with a multidisciplinary team, you will ensure timely referrals, data collection and alignment with the model of care, helping keep people in their usual homes when protected.
