Accoras is seeking a dedicated program lead for the Accoras Team-oriented Aftercare Program (ACAP). The role focuses on intake, partnership development and supporting the Service Manager to achieve program goals for youth with psychosocial needs.

You will conduct risk assessments, manage referrals and provide supervision to the team in a community-based setting, upholding high standards of care and collaboration with stakeholders.
