HomeMade is expanding its support team to empower older adults to live independently at home. You will manage caseloads of Home Care Package customers, guide families on self-management options, and tailor support plans within a cooperative, virtual-first workplace.

You will build trusted relationships, assess needs, and help clients navigate the HomeMade Portal and preferred providers, while working across onboarding, service, and clinical teams.
