Rise Child and Youth Services – Warwick QLD

Family Support Practitioner | Foster & Kinship Care | PPT & Casual We are inviting dedicated, practical and relationship-focused people to join our growing team across Warwick, Stanthorpe and the Southern Downs.

We are looking for people who understand that practical support, the right relationship, and someone turning up when they are needed can be the world of difference to a Carer Household.

Who we are:

- At RISE Child and Youth Services, we are dedicated to making differences in young lives through everyday interactions.

- The RISE approach builds resilience, empowerment and stability into the lives of children, young people and families by taking a personal approach to their needs, goals and circumstances.

- RISE currently provides a range of Child and Youth Services across the Toowoomba, Darling Downs and Southern Downs regions, including Transitional Care, Family and Carer Outreach, Foster and Kinship Respite, family contact, transport and intensive support.

- Our work is grounded in relational, trauma-informed practice and our ASCEND and DOSES frameworks.

Who you are:

- A calm, practical and supportive person.

- Someone who can build genuine relationships with children, carers and families.

- Emotionally intelligent, flexible and accepting.

- Comfortable working independently within the community as well as part of a broader team.

- Able to recognize when a family needs practical assistance, emotional support or additional help.

- Someone who can balance empathy with clear professional boundaries.

- Willing to ask, “what would it take?” and work alongside families to find practical solutions.

What sets you apart:

- Experience in Family Support, Youth Work, Child Protection, Foster or Kinship Care, Disability, Community Services or similar fields.

- Knowledge of the Queensland Child Protection system.

- Experience supporting families or households experiencing pressure, instability or complex circumstances.

- Grounded in local community and networks to help strengthen genuine supports.

- Strong communication, relationship-building and problem-solving skills.

- Confidence working with carers, children, government and community organizations.

- Individuals who can uphold our values of Respect, Integrity, Safety and Excellence.

The current role:

As a Family Support Practitioner with RISE, you will work directly alongside Foster and Kinship Carers, children and families to help maintain safe, stable and sustainable care arrangements.

No two families will need exactly the same thing.

One day may involve helping a carer manage appointments or transport. Another may involve providing direct support to children, assisting during a difficult period within the household, or working alongside other services to help prevent a placement from breaking down.

The role is deliberately flexible and responsive to the individual needs of each Carer Household.

RISE's proposed service includes planned and emergent support across business hours, evenings and weekends, with supports tailored to the needs of individual households.

Responsibilities may include:

- Providing practical and direct support within Foster and Kinship Carer households.

- Supporting children and young people within their home and community.

- Transporting children and young people to appointments, school, family contact or community activities.

- Supporting supervised family contact where required.

- Working with Carer Households experiencing increased pressure or placement instability.

- Helping carers identify practical strategies that strengthen household stability.

- Responding flexibly to changing family circumstances and emerging needs.

- Working collaboratively with Child Safety, Foster and Kinship Care agencies and other community services.

- Completing assessments, case notes and accurate service documentation.

- Contributing to individualised support planning and reviewing outcomes.

- Working within RISE's ASCEND and DOSES frameworks.
