ForHealth Medical Centres is seeking a Part time Chronic Disease Management Nurse for our Leichhardt Medical Centre. You will work with GPs and patients to prevent and manage chronic conditions, empowering patients to improve long-term health outcomes.
You will coordinate GPMPs/TCAs, conduct health assessments, educate patients, and coordinate referrals with a multidisciplinary team to enhance preventative care and reduce hospital admissions.
