About the Role

As a Homes First Case Manager at Junction Support Services, you will support individuals and families experiencing homelessness to secure and maintain safe, stable housing through Housing First, trauma-informed and strengths-based practice. You will build strong client relationships, assess needs, develop support plans, provide practical assistance and advocacy, and connect clients with relevant health and community services.

Working alongside the Homes First multidisciplinary team and external providers, you will promote client choice, self-determination and cultural safety, including supporting Aboriginal and Torres Strait Islander clients to maintain connections to culture and community. The role also involves maintaining accurate records, participating in supervision and professional development, and contributing to the after-hours on-call roster as required.

This is a full-time maximum term contract opportunity, based at our Wangarattauntil 30 June 2027.

What You Can Expect

In the role, you will:

- Deliver person-centred, trauma-informed and strengths-based case management that reflects Housing First principles and supports clients to achieve their goals and maintain reliable housing.
- Assess client and family needs and develop, implement and review individualised support plans,



including identifying the needs of accompanying children and responding to emerging risks and barriers.
- Support clients to secure and sustain long-term housing through practical assistance, advocacy, early identification of tenancy risks, and versatile, responsive support.
- Build strong partnerships and connections with clients, families, community services and the Homes First multidisciplinary team to coordinate supports and improve health, wellbeing and community connection.
- Maintain high standards of professional practice and accountability, including accurate case records, reporting, confidentiality, information sharing, participation in supervision and compliance with Junction policies, legislation and program requirements.

About You

You'll bring:

- A relevant tertiary qualification such as Social Work, Welfare or Behavioural Science and proven relevant experience to this role.
- Knowledge and understanding of the Housing First Principles
- Experience in providing holistic, strength-based, trauma informed case management support to vulnerable community members with multiple and complex needs.
- An ability to adequately assess individual needs in a family context, and develop, evaluate,



and update case support plans in relation to those needs using trauma informed practice.
- An understanding of the issues that may lead to a family or individual accessing a homelessness service.
