St Vincent's Private Hospital Toowoomba is seeking a Clinical Pharmacist to join the Hospital in the Home and Residential Inreach services in Melbourne. The role involves travel to patients’ homes and residential facilities to provide high-quality clinical pharmacy care as part of a multidisciplinary team.

You will manage medication histories, reconciliation, therapy reviews and counselling, ensuring protected, effective and timely use of medicines in home and community settings.
