Mercy Health Australia in Newcastle is seeking a Care Partner to join our Newcastle region-based team. This full time role focuses on delivering person-centred care management to enable older people to live safely and independently at home.
You’ll work within a rights-based framework, coordinating with participants, their supporters and interdisciplinary teams to plan, monitor and educate across Restorative Care, End-of-Life and AT-HM pathways, with emphasis on wellness and reablement.
