About the role

As a Care Partner, you'll work alongside our clients to understand what matters most to them and connect them with the supports that help them remain independent, safe and engaged in their community.

We've designed this role so you can spend more time building meaningful relationships and supporting our clients to live life on their terms, with the right support around you to help make that happen.

Think manageable caseloads, dedicated clinical guidance, admin support that actually supports, and teammates who'll happily jump in when you need them.

Key responsibilities

- Conducting Home Visits, completing assessments and ongoing reviews

- Developing and monitoring individualised care plans





- Managing participant budgets and funding responsibly

- Building trusted relationships with participants, families and service providers

- Coordinating supports and services

- Balancing participant choice, goals, risk and available resources to achieve great outcomes.

What you'll bring:

You might already be working in aged care case management or care coordination.... or you might be the person everyone comes to because you're calm under pressure, can juggle twelve things at once and somehow still remember everyone's dog's name!

Ideally, you'll have:

- Experience in aged care case management, care coordination or a similar role

- Confidence managing budgets and funding allocations

- Solid communication and relationship-building skills

- Excellent organisation and attention to detail
- An understanding of the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways
- A Qualification in Community Services, Nursing, Aged Care or a related field (desirable)

- A willingness to travel the Hervey Bay region for client visits and assessments using your own vehicle (kilometre reimbursement provided)
