About the role

As a Client Care Coordinator at Empowered Living Care, you will play a pivotal role in ensuring the highest quality of care is delivered to our participants. In this full-time position based in Caringbah, NSW, you will be responsible for coordinating and managing the care plans and support services for our clients, with a focus on promoting their independence, wellbeing and quality of life.

What you'll be doing

1. Developing and implementing personalised care/support plans in collaboration with clients, their families and our care team

2. Maintaining accurate participant records and ensuring documentation meets NDIS compliance standards

3. Support the onboarding of new participants to the organisation and liaise with them as key contact

4. Coordinating the provision of in-home support services, including personal care, domestic assistance, and social support

5. Regularly reviewing and adjusting care plans to ensure they continue to meet the changing needs of our clients

6. Communicating effectively with clients, families and our care team to ensure seamless service delivery

7. Monitoring and reporting on client progress and outcomes

8. Identifying and addressing any issues or concerns in a timely and professional manner

9. Participating in the continuous improvement of our care services and processes

What we're looking for

1. Relevant qualification in community services, disability support or a related field

2. Minimum 2 years' experience in a similar client coordination or case management role

3.



Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients and their families

4. Strong problem-solving and decision-making skills, with a focus on delivering the best outcomes for clients

5. Proficient in using client management systems and technology to support service delivery
6. A deep understanding and commitment to the principles of person-centred care

7. Ability to work collaboratively as part of a dedicated care team
