Bring your clinical expertise and compassionate, person-centred approach to a multidisciplinary team supporting people living with dementia across the community and residential aged care.
About This Role
Your new role
As an Enrolled Nurse, you will provide safe, evidence-based and person-centred care under the supervision of a Registered Nurse, supporting people with dementia and cognitive impairment who have complex care needs or behaviours of concern. You will work collaboratively with patients, carers and healthcare teams to identify triggers, implement individualised care strategies and help improve outcomes while supporting residential aged care staff to build their confidence and capability.
About You
You are a compassionate and capable Enrolled Nurse who is committed to delivering person-centred care and supporting positive outcomes for people living with dementia and cognitive impairment.
- Bring sound clinical knowledge and a commitment to safe, evidence-based nursing practice.
- Demonstrate, or have the ability to develop, skills in dementia care and managing complex behaviours.
- Recognise potential behavioural triggers and contribute to individualised care and behaviour management plans.
- Communicate effectively and work collaboratively with patients, carers and multidisciplinary teams.
- Demonstrate empathy, integrity and a commitment to continuous learning and professional development.

Community And Oral Health Services Community and Oral Health Services deliver a broad range of quality community-based care and support services to help people improve and maintain their wellbeing and independence as well as oral health services to adults and children. We deliver a myriad of services across Metro South including Hospital in the Home, Rehabilitation, Chronic Disease, Acute Care, Dementia Outreach, Oral Health, and specialist residential care along with delivering numerous other vital community health services.
Our services are delivered by multidisciplinary teams of nursing and allied health staff with the skills and experience to treat a full range of health needs.
Join the Dementia Outreach Service (DEMOS),
a multidisciplinary team supporting people living with dementia across the Metro South Health catchment in both community and residential aged care settings. The service provides early, responsive support for people with complex dementia-related needs and behaviours, while building the capability of aged care staff to provide safe, effective and person-centred care. Working collaboratively with patients, carers and healthcare teams, DEMOS supports individualised care and behaviour management strategies, promotes care in the community and helps reduce avoidable hospital presentations.
