We currently have an opportunity for a friendly and compassionate full time Home Carer to join our Community team.

The opportunity:

Reporting to the Home Care - Team Leader, the primary purpose of the Home Carer is to assist individuals in the community, providing person centred care and supporting optimum independence. Creating 'More Good Days'.

Responsibilities:

- Responsible for the provision of, and documentation of care and services provided.

- Responsible for the documentation of incidents and/or accidents, changes to Care Recipient details and circumstances, administration paperwork or other records as directed.

- Engage in Worker education, job planning, appraisals and feedback, team meetings and other activities that contribute to workforce and business development goals. This includes completing all designated mandatory training packages.

- Participate in quality improvement strategies by promoting Care Recipient and family feedback, engaging in research and development of projects as directed.





- Maintain regular contact with the Client to ensure a proactive approach to care coordination and to honour the level of Client involvement in management the home care package is achieved.

Essential Criteria:

- Certificate III in Aged Care or equivalent

- Police Certificate no older than 12 months at appointment.

- Current Drivers Licence.

- Experience working within a community care environment, with a focus on the provision of aged care services.

- Be able to work weekdays and weekends.

Personal Attributes:

- High level of interpersonal skills including demonstrating an ability to work with a broad range of clients, staff and stakeholders.

- Capacity to manage problem solving and dispute/ conflict resolution.

- Understanding of the Aged Care Quality Standards and other Government legislation that governs Community Aged Care Services.

- Aged Care experience in Residential facilities and Community settings.
