As a Wellbeing Practitioner, you'll play a vital role in supporting veterans and their families to overcome barriers, access the right services and build greater wellbeing, stability and connection. You'll work alongside people who have served our country, helping them identify their goals, navigate complex systems and create practical pathways forward.

About the role

Working within a supportive multidisciplinary team, you'll provide trauma-informed, strengths-based case management to veterans and their families experiencing a range of challenges.

You'll conduct comprehensive assessments, develop tailored support plans, coordinate services and advocate for clients to ensure they receive the support they need to achieve their goals.

Managing a caseload of approximately 15 to 20 clients, you'll build trusted relationships and work collaboratively with internal and external stakeholders to create positive and sustainable outcomes.

Key responsibilities

- Deliver trauma-informed, strengths-based case management services
- Conduct holistic assessments and develop individualised support plans
- Support clients to navigate health, housing, financial and community support systems
- Coordinate services and advocate on behalf of clients
- Assist veterans and families experiencing complex circumstances, including financial hardship and housing instability




- Build strong relationships with clients, community organisations and service providers
- Monitor client progress and maintain accurate case records and documentation
- Contribute to a collaborative and professional team environment

About you

You are a compassionate and skilled practitioner who is passionate about supporting people through challenging circumstances. You bring empathy, resilience and a strong commitment to achieving meaningful outcomes for clients.

To be successful, you will have:

- A degree qualification in Human Services, Social Work, Psychology, Health Sciences or a related discipline
- Eligibility for membership or registration with a relevant professional body such as AASW, PACFA or AHPRA
- At least two years' experience in a similar case management, counselling or wellbeing support role
- Demonstrated experience delivering strengths-based and client-centred support
- Exceptional interpersonal, communication and advocacy skills
- The ability to work effectively with people experiencing complex and sensitive issues
- Experience working collaboratively with multidisciplinary teams and external service providers

Experience working with the veteran community, Defence personnel or an understanding of the ADF environment will be highly regarded.
