This is an exciting opportunity to join Relationships Australia NSW in a flexible, community-based role supporting older people experiencing social isolation, chronic health conditions, or barriers to accessing services.
Working across Broken Hill and the surrounding region, you'll partner with healthcare providers, community organisations, local councils, and government stakeholders to strengthen community connections and improve wellbeing outcomes.

What You'll Do
- Support older people to build meaningful social connections and improve wellbeing.
- Conduct intake, psychosocial and risk assessments.
- Develop personalised wellbeing plans and connect clients with local supports and activities.
- Strengthen referral and community networks across the region.
- Build collaborative partnerships with healthcare, community and government stakeholders.
- Promote social inclusion,
community participation and positive ageing.
What You'll Bring
- Tertiary qualification in Social Work, Community Development, Social Welfare, Health Promotion or a related discipline.
- Understanding of mental health, aged care, social determinants of health, social prescribing and strengths-based practice.
- Strong stakeholder engagement, communication and relationship-building skills.
- Appreciation of the unique strengths and opportunities within regional and remote communities.
- Ability to work autonomously while collaborating effectively across diverse networks.
Essential Requirements
- NSW Class C Driver License.
- Access to a comprehensively insured motor vehicle.
- Ability to obtain a National Criminal History Check and Working With Children Check.
