Blue Butterfly Care is a clinically led NDIS provider supporting participants across South East Queensland. The organisation provides personalised disability support for individuals with a range of support needs, including Supported Independent Living, community participation, complex-care coordination and hospital-to-home transitions. Blue Butterfly Care works collaboratively with participants, families, Support Coordinators, allied health professionals, hospital teams and other stakeholders to promote independence, community inclusion and continuity of support.

About the Role

Blue Butterfly Care is seeking a Community Support Worker to assist participants in accessing community services, developing independence and achieving their individual goals. The successful candidate will work closely with participants and relevant stakeholders to identify support needs, implement individual support strategies, facilitate community participation and monitor progress in accordance with participant plans and organisational requirements.

Employment Type: Full-Time

Salary: $75,000 to $85,000 per annum plus Superannuation

Key Responsibilities

- Assess participants’ individual needs, goals and available community resources to identify appropriate support requirements
- Assist in developing and implementing individual support strategies that promote independence, participation and wellbeing
- Support participants to access community, recreational, educational and social activities in accordance with their individual goals
- Provide information and guidance to participants and families regarding available community services and support options
- Monitor participant progress and identify changes in support needs, risks or circumstances requiring further action
- Liaise with Support Coordinators, allied health professionals, healthcare providers,



community organisations and other relevant stakeholders
- Assist participants with referrals and connections to appropriate community, welfare and disability support services
- Support the implementation of recommendations from allied health professionals and participant support plans
- Maintain accurate progress notes, support records and reports relating to participant outcomes and service delivery
- Promote participant choice, dignity, independence and community inclusion in all support activities
- Identify and report safeguarding, wellbeing or service-delivery concerns in accordance with organisational and NDIS requirements
- Follow NDIS Practice Standards, privacy requirements, workplace health and safety procedures and organisational policies

Candidate Requirements

- Relevant qualification such as Diploma, Advanced Diploma or higher in Community Services, Disability, Welfare, Social Services or a related discipline
- Relevant experience of 1-2 years in community services, disability support, NDIS services or a comparable welfare-support environment
- Understanding of person-centred support, community participation and participant goal planning
- Ability to work effectively with participants, families and multidisciplinary stakeholders
- Solid communication, interpersonal and problem-solving skills
- Ability to maintain accurate participant documentation and progress records
- Understanding of participant dignity, choice, safeguarding and confidentiality requirements
- Knowledge of the NDIS and disability-support environment will be highly regarded
- Ability to work independently and effectively within a community-based support team
