The Royal Melbourne Hospital – Parkville VIC is seeking a Clinical Aged Care Assessor to join the Aged Care Assessment team. You will undertake strengths-based assessments across hospital, community, home and telehealth settings to determine care needs and facilitate access to Commonwealth-funded aged care programs and services.

As part of Melbourne's Northern and Western Metropolitan regions, you will develop care plans, complete reviews and make referrals to support the right care and services
