The Institute for Urban Indigenous Health (IUIH) is seeking a Regional Care Coordinator CCSS to implement the ITC program components with mainstream GP practices in South East Queensland. You will work with Aboriginal and Torres Strait Islander communities to support chronic disease care and care planning to achieve optimal health outcomes.
