Bendigo Health is seeking a Project Officer to support the Transition Care program in the Loddon region. The role focuses on reviewing referral pathways, occupancy and barriers, and developing a flexible service model to maximise program use and equitable access.

You will oversee data collection, KPI reviews, and produce reports with regular stakeholder engagement. Fixed term, part time until 30 June 2027, with 2026 Flu Vaccination required and a commitment to excellent care across Victoria’s
