Care Partner (Care Manager) – Support at Home

We are seeking an experienced and motivated Care Partner (Care Manager) to join our Support at Home Team.

This is an opportunity to work closely with older people and their families, supporting them to live safely, independently and well in their own homes through the delivery of personalised care and services. The role combines care management, service coordination, client advocacy and budget management within a supportive and collaborative team environment.

We are looking for someone who has:

Experience in aged care client care management, case management or service coordination

Robust relationship-building and communication skills

The ability to manage competing priorities and a diverse client caseload

Excellent assessment, care planning and documentation skills

Confidence working with client budgets and funding arrangements

A proactive and solutions-focused approach

A genuine commitment to supporting older people to live safely and independently at home

This position requires strong organisational skills, attention to detail and the ability to work both independently and collaboratively within a multidisciplinary team to deliver high-quality services and positive client outcomes.

Experience in home care, aged care or community care case management (specifically the Support at Home programme)

Strong knowledge of care planning and service coordination

Excellent communication and interpersonal skills

Strong documentation and record-keeping skills

Demonstrated ability to manage clients with complex or changing needs

Strong organisational and time management skills

Experience managing client budgets or funding packages

Ability to work autonomously and as part of a team





Sound computer skills including Microsoft Office and client management systems

Current driver's licence

Current NDIS Worker Screening Check (or ability to obtain)

About the Role

Managing a caseload of home care clients

Conducting client assessments and reviews

Developing and monitoring personalised care plans

Coordinating services and referrals to allied health and clinical providers

Supporting clients to achieve their goals while remaining within available funding

Monitoring client wellbeing and responding to changing needs

Liaising with clients, families, care staff and external providers

Maintaining accurate case notes, care plans and client records

Managing individual care budgets and service allocations

Participating in quality improvement, compliance and care review activities

Working collaboratively with care, quality, finance and administration teams

Other duties and projects as required
