St Vincent's Hospital Melbourne is seeking an experienced Clinical Pharmacist to join the Hospital in the Home (HITH) and Residential Inreach (RIR) services. You will deliver specialist clinical pharmacy care to patients in their homes and residential facilities, travel to patient sites, and work with a multidisciplinary team.

The role requires a recognised pharmacy qualification and current AHPRA registration, with at least three years of hospital experience.
