Location: Loch Sport and surrounding areas

Are you passionate about supporting older Australians to live independently at home? We are taking expressions of interest for a dedicated and compassionate Care Coordinator to assist us in getting a new project off the ground in Loch Sport.

As a Care Coordinator, you will play a key role in supporting clients to navigate their aged care journey. You will work closely with clients, families, and support workers to ensure services are tailored, responsive, and delivered with care and respect. Ideally, we are looking for someone who resides in Loch Sport or the surrounding area.

Key Responsibilities

- Coordinate and manage client care plans
- Conduct client assessments and regular reviews
- Liaise with clients, families, and service providers
- Monitor budgets and ensure services align with funding
- Support clients to achieve their goals and maintain independence
- Ensure compliance with aged care standards and guidelines

What You’ll Need

- Current National Police Check or will to obtain




- Valid Driver’s Licence and Reliable Vehicle
- Currently residing in Loch Sport or surrounds
- Experience in aged care, community services, or a similar role (desirable)
- Strong communication and interpersonal skills
- Highly organised with the ability to manage multiple clients in a fast-paced environment
- Confidence using Microsoft Office and ability to learn and use client management systems & care platforms
- Compassionate, client-focused approach
- Attention to detail and accurate data entry and record-keeping skills
- Knowledge of Support at Home or community care (desirable)
- Relevant qualification in Aged Care, Community Services, or similar (welcomed, but not essential)
