Your current role:

Lead specialised clinical and consultancy services across Metro South Health, tackling complex discharge barriers and hospital avoidance for patients with disability. Using advanced clinical expertise, stakeholder collaboration and service innovation, you'll influence pathways, strengthen cross-sector partnerships and lead sustainable models of care that improve patient flow and outcomes.

About you:

You're an experienced clinician and collaborative leader who can navigate complex clinical, system and cross-sector challenges to drive better outcomes for patients with disability.

- Bring advanced clinical expertise in complex discharge, hospital avoidance or a related specialty.
- Apply sound professional judgement to complex cases, clinical pathways and service challenges.
- Build strong partnerships across clinical teams, senior leaders, NDIS, legal and community stakeholders.
- Lead change, education, mentoring and evidence-based practice to strengthen workforce capability.
- Use data, research and innovation to improve clinical practice, service delivery and patient outcomes.
