Working closely with our Clinical and Operational Teams, you will help create positive outcomes while building meaningful relationships with participants and their support networks.No two days are the same in this diverse and fulfilling role.Key Responsibilities\*Clinical Care & Assessments\*Conduct participant assessments, reviews, and ongoing monitoring\*Assist in developing and reviewing support plans and care plans\*Complete clinical, behavioural, and manual handling risk assessments\*Support medication management and clinical care requirements\*Assist with wound management, PEG feeding, catheter care,
and other clinical supports within your scope of practice\*Care Coordination & Stakeholder Engagement\*Collaborate with participants, families, support coordinators, allied health professionals, and medical practitioners\*Assist with coordinating services to ensure participant goals and health outcomes are achieved\*Support participants and families throughout their care journey\*Staff Support & Education\*Provide clinical guidance and mentoring to Disability Support Workers\*Deliver staff inductions, training, and competency assessments\*Promote best practice and person-centred approaches to care\*Quality & Compliance\*Contribute to clinical governance, quality improvement initiatives, and audits\*Ensure clinical documentation remains accurate and compliant\*Support the organisation's commitment to continuous improvement and participant safetyAdditional Responsibilities\*Participate in an on-call roster as required\*Support business growth through clinical expertise and participant advocacyAbout YouYou are passionate about making a genuine difference in people's lives and thrive in a supportive,
