Junction Support Services invites applications for a Homes First Case Manager in Wangaratta, VIC. You will support individuals and families experiencing homelessness to secure and maintain safe housing through Housing First, trauma-informed, and strengths-based practice.

You will build robust client relationships, assess needs, develop support plans, provide advocacy, and connect clients with health and community services, including after-hours on-call roster as required.
