EZ Home Care Pty Ltd is seeking a compassionate, reliable and experienced Disability Support Worker to join our team across Perth, Western Australia. (Multiple positions are available across these locations. Successful applicants will be placed at one of these locations depending on operational requirements.)

The successful applicant will provide person-centred support to people living with disability, assisting them to live as independently as possible and participate in their home and community life.

About the Position

As a Disability Support Worker, you will provide practical, personal and emotional support to clients according to their individual needs, support plans and goals.

Key responsibilities

- Deliver person-centred care and daily living support aligned with NDIS standards.
- Assist participants with personal care, meal preparation, mobility, and medication support.
- Promote social inclusion and community participation activities.
- Maintain accurate documentation, incident reports, and progress notes.
- Liaise effectively with clients, families, and allied health professionals.




- Uphold workplace health and safety and organisational compliance standards
- Assist participants with travel and transport to appointments and community activities.

Skills and Qualifications

- Certificate III or IV in Individual Support (Disability, Aged Care, or Community Services) or equivalent qualification.
- Minimum 1 year of relevant experience in disability or community services (preferred).
- Current First Aid and CPR certification.
- National Police Clearance and Working with Children Check (mandatory).
- Valid Australian driver’s licence and access to a reliable vehicle (desirable).
- Excellent interpersonal and communication skills with a caring attitude.
