About Spinal Community Services

The VSCS is a specialised state-wide service that offers lifelong care to people who sustain a traumatic spinal cord injury (SCI) in Victoria, Tasmania and southern New South Wales. Inpatient services are provided at the Austin and Royal Talbot campuses of Austin Health. The VSCS also provides community and outpatient programs.

VSCS Spinal Community Services includes the Spinal Community Integration Service (SCIS), a community facing service supporting clients establish themselves in the community following spinal cord injury rehabilitation, the Spinal Outreach Service (SOS) supporting clients manage common complications of spinal cord injury that can lead to significant health issues and re-hospitalisation, and several outpatient clinics offered to assist clients proactively manage their SCI related health.

About the role

You will work as part of the interdisciplinary SCIS team and in collaboration with inpatient and ambulatory teams and community agencies, will provide nursing services and consultation to clients recently discharged from rehabilitation.

A regular day will see you:





- Assisting the inpatient team with discharge planning
- Facilitating and coordinating the optimal community integration for SCIS clients
- Assisting clients with community integration goal setting and review
- Educating clients regarding their injury and available services
- Assisting clients to access local services
- Supporting community providers with high level SCI knowledge
- Educating community providers on SCI topics
- Empowering clients to manage their own healthcare by the end of their SCIS episode

About you

To be successful in this role you’ll bring with you:

- AHPRA registration
- Clinical experience in the area of spinal cord injury
- An interest in working in the community setting
- An ability to motivate, mentor and inspire
- A collaborative spirit, and ability to build strong networks with peers
- Excellent communication skills
- Enjoyment in working with a diverse multi-disciplinary team
