The Role
We are seeking an experienced and highly motivated Planning Business Partner to work closely with participants, their families and support networks to help them achieve their goals and improve their quality of life.
This role is central to participant intake, service planning and ongoing engagement across CLA's disability services. Working closely with the Service Delivery Business Partner, Coordinators and the Community and Business Development Officer, you will help ensure supports are responsive, person centred, financially sustainable and aligned with each participant's goals. You will also contribute to marketing, referral development and business growth activities that strengthen CLA's presence across the Great Southern.
Duties:
- Complete intake assessments, risk assessments and service agreements for new referrals.
- Support participants to identify, choose and manage their lifestyle routines and make informed decisions about their supports.
- Work closely with the Service Delivery Business Partner and Coordinators to monitor participant needs, goals, service quality and changing risks.
- Monitor plan outcomes, service utilisation and expenditure to support participant outcomes and ensure services remain within available funding.
- Build trusted relationships with participants, families, nominees, support coordinators and other stakeholders while maintaining appropriate professional boundaries.
- Develop and maintain referral networks with local agencies, health professionals, support coordinators and community organisations to identify suitable service opportunities.
- Represent CLA at expos, interagency meetings and provider networking events, promoting CLA's services in a skilled and accurate manner.
- Contribute to marketing and business development activities, including campaign ideas, community engagement,
lead follow up and the conversion of suitable enquiries into sustainable services.
- Collaborate across CLA to improve participant experience, service quality and responsible growth.
We are looking for someone who has:
- Experience in participant planning, service planning, support coordination or a similar role within the disability, community services, human services or allied health sector.
- A good understanding of the NDIS and individualised funding, including the ability to monitor plan utilisation and expenditure.
- Experience working directly with participants and families to understand their goals, needs and support requirements.
- Strong relationship-building and communication skills, with the confidence to work with participants, families, Support Coordinators and other stakeholders.
- Good organisational skills and the ability to manage competing priorities.
- Sound understanding of the PRODA system and the NDIS Pricing Arrangements and Price Limits.
- Experience in community engagement, referrals or business development would be an advantage.
