About the role

We are seeking a Registered Nurse to work alongside our Clinical Care Coordinator conducting hands-on clinical assessments and follow-up reviews directly with our consumers, in their own homes. This is a part time role with flexible hours, well suited to an RN who wants meaningful clinical work without a rigid weekly roster. You will coordinate your own assessment visits directly with consumers and their families, working within our clinical governance framework and reporting clinical findings back to the Clinical Care Coordinator for care plan sign-off.

Key responsibilities

- Coordinate and conduct initial, annual, and triggered comprehensive clinical assessments directly with consumers in their homes
- Carry out follow-up clinical reviews where a consumer's needs, risks, or circumstances have changed
- Assess and document mobility, falls risk, medication management, wound care, continence, pain, cognition, and nutrition as part of each assessment
- Liaise directly with consumers, families, and GPs to coordinate assessment visits and follow-up care needs
- Accurately document all assessment and follow-up findings in our CRM system (Sandwai) within required timeframes




- Escalate any clinical risk or safety concern identified during an assessment to the Clinical Care Coordinator promptly
- Work collaboratively with the Clinical Care Coordinator, who retains overall clinical governance and care plan sign-off

About you

- Current AHPRA registration as a Registered Nurse (mandatory)
- Experience conducting comprehensive clinical assessments, ideally in aged care, community, or home care settings
- Comfortable coordinating your own visit schedule directly with clients and working with a good degree of independence
- Strong clinical documentation skills and confidence using a CRM system for records
- Warm, respectful, person-centred approach to working with older people in their own homes
- Current police check (or willingness to obtain one) and a valid driver's licence for home visits across metropolitan Melbourne
