You will deliver holistic, person-centred care within clients' homes as part of a multidisciplinary team, drawing on primary health care principles to support chronic conditions and complex needs.

The role emphasizes direct clinical nursing, early intervention, health education and recovery-focused support, with a Monday–Friday schedule and no night shifts.
