Reports To:ManagementPosition Summary:The Home Care Worker provides person‐centered support to older people living in their own homes, assisting them to maintain independence, dignity, and quality of life.
The role involves delivering practical, emotional, and personal care in line with individual care plans, organisational policies, and aged care standards.Key Responsibilities:Personal Care:Assist clients with activities of daily living, including showering, grooming, toileting, dressing, and mobility.Support safe transfers and use of mobility aids as required.Promote comfort, dignity, and respect at all times.Domestic Support:Assist with light household duties such as: Cleaning and tidyingLaundry and ironingMeal preparation in accordance with dietary requirementsSupport clients to maintain a safe and hygienic home environmentSocial Support & Companionship:Provide companionship and emotional supportEncourage social engagement and participation in community activitiesAccompany clients to appointments or outings when requiredHealth & Wellbeing Support:Observe and report changes in client health, behaviour,



or wellbeingPrompt or assist with medication according to care plans (where authorised)Follow infection control and work health and safety proceduresDocumentation & Communication:Complete care notes accurately and in a timely mannerCommunicate effectively with supervisors, care coordinators, clients, and familiesAdhere to confidentiality and privacy requirementsSkills and AttributesCompassionate, respectful, and client‐focused approachAbility to build rapport and trust with older peopleStrong communication and interpersonal skillsAbility to work independently and manage time effectivelyUnderstanding of qualified boundariesCultural awareness and sensitivityQualifications & Requirements:Certificate III in Individual Support (Ageing) or willingness to obtainCurrent First Aid and CPR certificate (or willingness to obtain)National Police CheckWorking with Children Check (where required)Current driver's licence and reliable vehicle (preferred)Evidence of the right to work in AustraliaDesirable Experience:Previous experience in aged care, home care, or community servicesExperience supporting clients with mobility issues, dementia, or complex needsKnowledge of Home Care Packages and person‐centred care principlesOther Requirements:Ability to follow care plans and organisational policiesCommitment to professional conduct and continuous improvementWillingness to undertake training and professional development
