Mid North Coast Local Health District in Port Macquarie invites applications for a Social Worker (Level 3) in the Transition Aged Care Program.
You will provide high-quality social work services, clinical leadership and care coordination as part of the Hastings Macleay Clinical Network to support older people after hospital discharge.
You will work with a multidisciplinary team, contribute to continuous quality improvement, and must hold current NSW Driver Licence with flexibility to travel as
