Silverchain is hiring a Care Partner to support older Australians in the Kinglake West area.
You will assess needs, coordinate services and advocate for protected, independent living at home.
In this full-time role, you will visit clients in their homes, build meaningful relationships and work with clinical, scheduling and service delivery teams to deliver coordinated care.
You'll manage a caseload with autonomy and flexibility to plan your day.
