Job Description
Mercy Health in Werribee invites a Social Work / Care Coordination skilled to join the Health Independence Program team.
Based in a dynamic office, you will support clients in the community and outpatient settings with assessments, crisis intervention, and referrals.
The role requires a Bachelor or Masters of Social Work, eligibility for ASW, and police/WWCC checks.
Full Victorian Driver's Licence and experience in advocacy, mental health, housing or elder care are highly regarded.
