Job Description
Primary Care Connect is seeking a Health Navigator to undertake initial health screening for children and young people in care, coordinating health information and referrals across Shepparton and Seymour.
The role supports trauma informed, culturally protected care and works with Child Protection and Out of Home Care workers.
Reporting to the PCC Nursing team, the role is based in Shepparton, with a focus on cross-sector collaboration to ensure timely access to health services and management plans
