Life Without Barriers is seeking a Hub Home Provider in Perth (North Suburbs) on a maximum term, part time contract ending 31 December ****.
You will support Satellite Families and carers, providing a welcoming Hub Home for connections and practical assistance.
You will coordinate meetings, support placement challenges, connect families with education and health services, and maintain accurate records while building strong relationships with carers and young people.
