Job Description
Whiddon is seeking a Community Care Coordinator to lead social and care networks for older Australians in NSW/QLD.
You will oversee client care plans, conduct assessments and set goals to promote independent living in the community.
Ideal candidates have leadership experience, knowledge of aged care compliance, and may be RN with AHPRA (not essential).
This is a fixed-term, full time role based in Tamworth, with field visits to meet clients and network.
