ForHealth Bankstown NSW is seeking a Permanent Full time Chronic Disease Management Nurse to lead preventative care efforts within a busy practice. You will work with GPs to identify and manage chronic conditions and keep patients out of hospital.
Join a values-driven health network and contribute to multi-disciplinary care plans and ongoing patient support. AHPRA registration and experience in CDM are essential.
