Whiddon is seeking a Community Care Coordinator to lead social and care networks for older Australians in NSW/QLD. You will oversee client care plans, conduct assessments and set goals to promote independent living in the community. This is a fixed-term, full time role based in Tamworth, with field visits to meet clients and network.
