About the Role: ACH Group have a 0.8 FTE part time – fixed term opportunity until 5 March **** , within the Home Care Community program based at the North office.
What you'll do: Support customers to identify preferences and connect with social experiences at ACH Group and/or in the broader community work with the Social Community Team to promote and grow the Social Link experiences match customers to relevant social experiences including ACH social experiences and wider social opportunities using a person directed model follow-up customer referrals and support recent customers to feel comfortable joining groups or connecting with individual support – assist customers to overcome barriers to participation provide time-limited individual planning, support coaching to complex, high risk customers to meet social goals and discuss risk management plans with Care Manager.
